    “Isn’t it strange to NOT have a letter of recommendation from the academic/research advisor, even if that person did not supervise the student clinically?”


    “Would you ever recommend soliciting a letter of recommendation (potentially as your 4th letter) from a research mentor who knows you well (and is well known in the field) who can speak to your ideas/ways you think about clinical research etc. even though they haven’t supervised you clinically?”

    I think that it makes sense to have a letter from a research mentor even if that person does not provided clinical supervision. As you say, this person presumably knows you well and can comment about your ways of approaching clinical issues.
